Answer:
Roof pitch is only needed for flashings that are installed on the roof itself, as there are a few different flashings available. These cathedral ceiling support boxes are made to be installed into ceilings of any pitch. They are available in two sizes: 24" in height, like this one, or 11" in height, like the one found at the following link:

I have a 4/12 pitch roof, 12" rafters on 24" centers with a pro-panel roof, 12 ft. from stove to ceiling and 3 ft. outside. Do you have a kit that includes all needed pipe and acc. and will this box work well here.

Answer:
Per your dimensions, this cathedral box will work well for your application. We unfortunately do not offer a stove pipe component kit. However, we'll gladly build a chimney system quote for you. We've built a rough quote and emailed it to you. Per your roof pitch, it is recommended that you have just over 3' of pipe above your roof. In the quote, we assumed that you have no real attic space to mention. We did include enough pipe to accommodate this cathedral box, though. We also assumed that you'd prefer single wall stove pipe. We can easily alter any of these components, if you'd like.
