Can you confirm that the insert can operate without a ceramic liner? Is the ceramic liner required for use of just an option for appearance?
The liners are not required for the fireplace to be safe and operational. The liners are purely for aesthetics, in this model of insert. The stock interior is satin black.

Does this have glass doors?
Yes, this insert, as with all direct vent units, features a solid glass front. Please note this insert is only suitable for installation inside an existing fireplace (usually brick/masonry) with a functional chimney in place.

I purchased the flex kit #DVK12P-1 for this fireplace. I'm in the middle of installation the instructions mention using CO-linear connectors / flex coupling. Are these items necessary for installation?

Looking at the manual, I'm trying to find the combustible clearances for the inside of the existing fireplace box. Are they zero?
This unit must be installed within a fully non-combustible existing, functional fireplace with chimney. If that is the case and all dimensions for sizing are met, then there is no clearance required otherwise.
