Can you add an electrical outlet to the 1800 insert to run the fan? I really would not like to run the blower fan cord outside the unit.

Depending on your local electric code, you may or may not be able to install a junction box within the fireplace so that the blower can be plugged in behind your flashing/surround.

Is this a variable speed fan? If so, does it operate by thermostat or is it manual speed controlled? Lastly, is this a quiet fan?

This is a variable-speed blower, and it is manual controlled with a rheostat. A rheostat is similar to a circular light switch dimmer where you can control the brightness of the light. A similar switch is on this blower that controls the speeds. It is a relatively quiet fan, although of course it is going to make some noise.
