No this is for a vertical installation. You need a wall thimble to go through the wall.

can I use this ceiling support on a slightly pitched ceiling (not quite flat, but a few degrees of pitch)

You could if the pitch is below a 1/12. To maintain the proper pipe clearance you should use Cathedral Ceiling Support Box. This allows the stove connector pipe to maintain the correct clearance from the sloping ceiling.
