What is included with the fireplace?

The firebox and smoke chamber are included, but all the stone veneer, the hearth, and the chimney cap would be purchased/obtained separately. You can get a better idea about what the actual unit is as shipped to you by looking at the installation manual here.

What all is included in price relative to chimney height, fire brick lining, and mortar needed for construction and fire brick for primary unit?

This price includes 2-3 feet of masonry chimney, but it does not include firebrick or mortar. It also does not include the finishing veneer material shown in the main photo.
