What are the exact measurements? Can I connect this straight to the insert?

The narrow end is 6" in diameter and is designed to adapt straight to the appliance. The larger end is 6 5/16" in diameter to connect straight to the chimney liner.

Is there an adapter to join 6-inch DuraFlex 304 to 6-inch DuraPlus triple wall or Duraliner double wall?

We do not have a listed adaptor/transition from DuraFlex to DuraPlus. The only similar option would be for the DuraLiner flexible chimney pipe.

No, you need a transition anchor plate to make the conversion from stainless steel liner to rigid class A pipe. The exact part you need will depend on the model of class A chimney pipe you will be using.
