That part is actually inside the firebox, sloped from front to the back, basically the "ceiling" of the firebox.

Linear direct vent fireplaces such as this model tend to be more focused on style and aesthetics as opposed to straight-up efficiency.  This model would not be considered high-efficiency.  The high-efficiency direct vent models we sell are more in the traditional style, with louvers (vents) above and below the glass front and a log set instead of glass/stones.

Yes.  We would recommend either recessing the television or installing a mantel shelf above the fireplace, with the television above the shelf, not protruding out over the front of the fireplace.
