Will I need to install some kind of vent pipe to fit this unit into a natural fireplace?

Yes, you will need to run a vent kit consisting of two 3" diameter liners up through the existing chimney, all the way to the top. You can see the vent kit for this particular insert here.

What is the efficiency rating of this GDIX4? Can it operate without power in an emergency?

The efficiency rating of this unit is 66% and this unit can turn off/on and adjust flame height through the battery powered receiver during a power outage. However, you will not be able to work the blower or night light features.
