How high above this fireplace should the TV be mounted?
The manual states that a TV should be mounted a minimum of 12" above the fireplace in a flush-wall application, and 18" if the TV is recessed. They also recommend using a mantel or HeatFlo Kit when mounting a TV above the fireplace. You can read more on the installation specs here.

If the damper frame has a 6-inch opening width, does the exhaust pipe have to be double-walled all the way up to the top of the chimney?
If the damper frame has a 6-inch opening width, does the exhaust pipe have to be double-walled all the way up to the top of the chimney?
Yes, when installing a full linear fireplace in an application such as yours, you will need enough room to run the double-wall pipe up the existing chimney.  Most models will need at least 7.5 inches of room for that vent pipe, including this Superior DRL3500 55" model.  As such, the only way for something like this to work for you is to remove that damper and then run the vent pipe up through the damper shelf to the top of the chimney.

Real nice linear fireplace

We are very happy with the 55” Superior fireplace and the service we received along with it. We installed it on our new porch. Makes good heat and the flame adjustment was simple. I prefer it over our traditional gas fireplaces.

Pros

Linear just works better especially on our porch.
