Does this fit into a 6" stove pipe requiring drilling for the damper to rotate?

You would indeed need to drill into the stove pipe to allow for the damper handle to rotate.

How far above the stove should the Damper be set?
    1 Answer
Most manufacturers will specify the ideal height, but in the event that you are using an older stove that does not say, 12 inches is a good height. This will still allow it to function properly without you having to put your hand and arm too close to the hot stove top.

How do you get the pin out? And install?
    1 Answer
The damper pin will have a small bend nearer the handle, which will hold the pin securely within the cast iron plate. In order to free the pin, you must push the handle in and compress the spring fully. This will force the bend in the pin out of the retaining sleeve on the plate. You can then rotate the pin and pull it out of the plate. It will be necessary to hold the plate while performing this procedure so that it does not bend the stovepipe or fall into the stove when the pin is removed.
