Is the vertical dimension 28 1/4" measured at the top of the trim or the firebox housing itself?

The 28.5" measurement is for the rough-in framing dimensions, the trim panel measures 28" high as outlined in the installation manual.

What is the estimated area the 240 V. is designed to heat?

Per the manufacturer, the 240v wiring option could heat a space of approximately 400 square feet with standard 8' ceilings.

Please explain the following: 120 or 240 volts, does 240 have a larger flame? Also, the BFSL33DOOR kit is a glass front. The fireplace has a glass front, why would you need the kit?

The door kit is a set of faux doors to be used with your existing glass front. The voltage difference is simply whether you are going to hardwire or plug in, so there will be no change in the unit's performance (or flame).

This will include a steel mesh curtain and trim, but the partially reflective glass is set back from the mesh.
