When running at its highest speed, there will certainly be a noticeable humming noise, though I would not consider it to be overly noisy.

Is there an off switch for the blower?
The variable speed switch, aka rheostat, included with the FBB10 will provide On and Off control in addition to adjusting the speed, but please note that this blower is only approved for certain Empire fireplaces.

No. The correct blower for a DVD36FP30 is the FBB4.

No, the blower for that Tahoe would be the FBB-4.
