While it is not impossible for a DIY replacement, it is best to have a mason do this style of repair to support any masonry material that the damper frame is supporting.

Fireplaces are designed to be operated with the throat damper wide open. It should only be tapered to a closed position as the fire dies down to coals.

No, a damper on a woodburning fireplace should always be fully open to prevent smoke and carbon monoxide intrusion.

A 7" damper plate likely wouldn't work because it wouldn't have the properly sized hinges to hang onto your space.
