Are these Vestal dampers supposed to stay open a 1/4 inch due to a small cast-in "bose" that bumps against the edge of the throat frame?

Per Vestal, it should not have a gap. If there is something preventing it from shutting they suggested trying to grind it off.

Do we need any accessory to complete the damper?

Besides any mortar you would use to affix the damper assembly into the throat of the chimney, this assembly includes all parts necessary for installation.
